Important Notice
This form will create a new booking record and send an email confirmation to both the customer and instructor. For paid services, a payment request email will also be sent to the customer with a secure link to confirm their payment method.
⚠ Do NOT use this for Full-Time Enrolled students unless they specifically request an extra lesson outside of their enrollment.
